AI Business Insights for Patient Flow at a Healthcare Clinic

AI Business Insights for Patient Flow at a Healthcare Clinic
Case Study

Overview

Praxis Dr. Wenger & Partner is a family practice in Winterthur with three doctors, two practice assistants, and one medical practice assistant. The practice serves approximately 2,500 active patients and conducts an average of 60 consultations per day. Established for 15 years with a good reputation in the region, the practice increasingly struggles with waiting times, patient dissatisfaction, and inefficient utilization.

Background

Client Context

Practice manager Dr. Claudia Wenger knew intuitively that scheduling was not optimal but could not prove it. Monday mornings and Tuesday mornings were regularly overbooked, Thursday afternoons and Friday afternoons were often half empty. Appointment slots were uniformly set at 20 minutes, regardless of whether it was a brief follow-up or an extensive initial examination. The consequence: with long consultations, subsequent appointments backed up, and waiting time could exceed 45 minutes.

Challenge

The Problem

The problems could be quantified: average waiting time was 28 minutes, with outliers exceeding 60 minutes. The no-show rate was 12%, with certain patient groups (younger patients, Monday afternoon appointments) showing significantly higher rates. Actual consultation duration varied widely: 8-15 minutes for follow-ups, 25-40 minutes for initial examinations, 15-25 minutes for chronic conditions. The uniform 20-minute slot fit none of these categories well. Result: stress for doctors and practice team, frustrated patients, and revenue loss from idle time.

Approach

Our Solution

We implemented an AI Business Insights system that analyzed the practice data from the last 3 years and provides three core functions: First, intelligent appointment schedule optimization that assigns different slot lengths based on the probable consultation type (10, 20, or 30 minutes). Second, utilization forecasting that predicts expected patient flow per day and time of day and provides staffing recommendations. Third, a no-show risk model that calculates the probability of non-attendance per appointment and automatically sends SMS reminders or sets shorter confirmation deadlines for high-risk appointments.

Execution

Implementation Highlights

The particular sensitivity of patient data required strict data protection measures. All data was processed locally on the practice server. Nothing left the practice. AI models were trained on anonymized data and work only with statistical patterns, not individual patient information. Integration with the existing practice information system (PIS) was done through a secure interface that only exports aggregated appointment data. An important detail: the AI provides recommendations, but final appointment scheduling always remains with the practice assistant. The system does not replace human decision-making but supports it.

Impact

Key Results

Results after 4 months are impressive: average waiting time dropped from 28 to 18 minutes, a 35% reduction. The no-show rate decreased from 12% to 7%, mainly through targeted SMS reminders for high-risk appointments. The practice can now treat 20% more patients per day because afternoon idle times were filled and morning overloads were balanced. The Google rating rose from 4.1 to 4.8 stars. Several new reviews praise the 'short waiting times' and 'punctual appointments.' Practice assistants report that intelligent scheduling has significantly eased their daily work.

Takeaways

Key Learnings

Key insight: the biggest impact came not from the most complex feature (demand forecasting) but from the simplest (variable appointment lengths). Just switching from rigid 20-minute slots to variable 10/20/30-minute slots reduced waiting time by 20%. Forecasting and no-show prevention delivered the additional 15%. In healthcare, data privacy is not a nice-to-have. It was the basic prerequisite for the doctors' trust. Local data processing was more expensive than a cloud solution but essential for acceptance.
